We currently collect and process the following information:

  • Assessment and Diagnostic details

  • Recordings of virtual assessments

 

How we get the personal information and why we have it

Most of the personal information we process is provided to us directly by you (or a parent/carer) for one of the following reasons:

  • Providing assessment services
  • Improving our service
  • Part of our normal business

We also receive personal information indirectly, from the following sources in the following scenarios:

  • Contributions to the provision of assessments from persons authorised to do so by you (or a parent/carer).

By consenting, your personal information will be used for the purpose of providing your requested assessment and the improvement of our service and will be accessed solely by professionals involved in this service provision.

We may share this information with organisations or people authorised by you (or a parent) AND any organisation or person identified by us, whether authorised by you or not, for the purpose of safety towards yourself or others or where required to do so by law.

Under the UK General Data Protection Regulation (UK GDPR), the lawful bases we rely on for processing this information are:

How we store your personal information 

We keep all personal information stored securely in a password-protected encrypted database on a Microsoft 365 system. No paper-based data is stored. All records will only be retained for the minimum time necessary. We will then dispose your information by digital deletion when no longer necessary and by a maximum of 6 years after the end of your relationship with ADHD Complete LTD.

Under data protection law, you have rights including:

Your right of access - You have the right to ask us for copies of your personal information. 

Your right to rectification - You have the right to ask us to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete.

Your right to restriction of processing - You have the right to ask us to restrict the processing of your personal information in certain circumstances. 

Your right to object to processing - You have the the right to object to the processing of your personal information in certain circumstances.

Your right to data portability - You have the right to ask that we transfer the personal information you gave us to another organisation, or to you, in certain circumstances.

You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.
